Closed Without Verification
Administrative closure to meet a deadline resulted in unverified errors.
Case Lifecycle & Protocol Analysis
During multidisciplinary drawing coordination, tracking comment integrity across architectural, structural, and mechanical sets requires strict accountability. When reviews are marked resolved without sufficient verification or cross-sheet synchronization, downstream conflicts escalate into costly field modifications.
In this project phase, sixty pending markups were administratively updated to 'Closed' forty-eight hours prior to a major milestone submission. The design team assumed that verbal promises and tentative draft notes were already integrated into the production sheets. Upon site mobilization, concrete formwork was poured according to outdated penetration schedules, triggering immediate structural RFIs and concrete coring delays.
Key Finding & Observations:
- Response captured the symptom but lacked underlying coordinate synchronization.
- Closure was approved prematurely before republishing correlated drawing layers.
- Audit trail remained ambiguous regarding primary discipline responsibility.
- No second-party QA check was completed to verify physical drawing clouding.
