In this project scenario, the design team adjusted the shaft dimensions on sheet A-102 to accommodate larger service runs. The comment was marked resolved immediately after updating the partition layout. No cross-notification was dispatched to the structural engineers responsible for slab openings on S-201, leaving the structural model misaligned with real architectural geometry.

Key Finding & Observations:

  • Response captured the architectural layout change but omitted structural opening re-calculations.
  • Closure was approved unilaterally by a single discipline coordinator without secondary verification.
  • The comment status switched to Closed in the review session while the correlated structural drawing remained in an outdated state.
